Job summary

Elara Caring is seeking a Medical Social Worker (MSW) to join our home health team in the United States. You will assess patients’ psychosocial, financial, environmental, and safety needs, develop care plans, and coordinate with physicians and the care team to support positive outcomes.

You will connect patients and families with community resources, counsel on social and emotional needs, and facilitate planning for safe discharges.

Qualifications

• Current State License as a Social Worker.
• Master’s or Doctoral Degree in Social Work from a CSWE-accredited school.
• 1+ year of social work experience in a healthcare setting.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Assess psychosocial, financial, environmental, and safety needs.
• Complete initial social work evaluations and reassess during follow-up visits.
• Develop and update individualized care plans based on patient needs and physician orders.
• Communicate changes in patient condition to physicians and the care team.
• Connect patients with community resources and support services.
• Counsel and educate patients and families on social, emotional, and care needs.
• Coordinate with the care team for continuity of care and discharge planning.
• Provide compassionate social work services to support positive patient outcomes.
