Job summary

Calvary Hospital in Brooklyn, NY is seeking a Hospice Social Worker to deliver medical social services, psychosocial and bereavement counseling for patients and their families. A MSW is required with LMSW/LCSW eligibility preferred; driving may be required for patient visits in the tristate area.

Preferred candidates have at least one year of bereavement counseling experience and hospice/home care exposure. The role supports compassionate end‑of‑life care within our Calvary Hospital system.

Qualifications

• The Hospice Social Worker provides medical social services, psychosocial and bereavement counseling to patients and families.
• Valid driver's license may be required; a vehicle may be needed to commute to patients.
• Experience in hospice or home care is preferred.
• At least one year bereavement counseling experience is preferred.

Qualifications

• Minimum one (1) year bereavement counseling experience is preferred
• At least one year of Hospice or Home Care experience preferred
