Job summary

Transitions Hospice - Illinois is seeking a Medical Director to provide overarching medical leadership for the Springfield program, serving as the clinical authority and quality steward across hospice services.

You will supervise providers, coordinate pain and symptom management, ensure 24/7 physician coverage, develop evidence-based protocols, and promote education and community engagement with hospice partners. This role joins a mission-driven team dedicated to compassionate end-of-life care.

Qualifications

• Licensed as a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) in Illinois without restriction or disciplinary action
• Maintains controlled substances registrations with state and federal authorities
• Not excluded from participation in the Medicare Program
• Knowledge and prior experience in hospice, palliative care, and/or end-of-life care
• Commitment to ongoing medical education in hospice and palliative care
• Strong clinical judgment in pain and symptom management for terminally ill patients
• Ability to serve as an educator, collaborator, and advocate across interdisciplinary teams
• Genuine commitment to hospice philosophy and values

Responsibilities

• Provide overall medical leadership for the Transitions Hospice program as clinical authority and quality steward
• Supervise hospice providers and consult with attending physicians on pain and symptom management
• Serve as primary medical resource to the hospice interdisciplinary group
• Ensure 24-hour physician coverage to meet medical needs across hospice patients
• Review and develop treatment protocols and evidence-based intervention options
• Maintain knowledge of hospice/palliative care trends and research
• Collaborate as collaborating physician for practitioners within the organization
• Establish and review policies related to patient care, education, and emergency protocols
• Ensure adherence to Medicare Conditions of Participation and CHAP standards
• Evaluate QAPI plans and participate in related teams and activities
• Develop criteria to monitor education programs and outcomes
• Assist with evaluation of protocols with respect to quality and cost outcomes
• Liaise with physicians for consultation and education within hospice
• Represent and promote the program to physicians and referral sources
• Support programs addressing medical education needs
• Counsel and communicate with patients and families navigating end-of-life care
• Engage in ongoing medical education activities
