The Staff Pad is partnering with a leading hospice and palliative care provider in the Dallas–Fort Worth area to hire a compassionate Master Social Worker (MSW, LMSW, or LCSW) in Denton, TX. The MSW will conduct psychosocial assessments, counsel patients and families, coordinate community resources, and support end-of-life care within an interdisciplinary team.

This full-time role requires Texas licensure and healthcare experience; we offer competitive salary, benefits, PTO, and education

Qualifications

• Master's Degree in Social Work (MSW) required.
• Texas LMSW or LCSW license required.
• Minimum one year of healthcare social work experience.
• Hospice/palliative care experience preferred.

Responsibilities

• Conduct psychosocial assessments to identify patient and family needs.
• Provide counseling and emotional support to patients and families.

Reporting to the Director of Family Care Services,The Master Social Worker provides coordinated hospice care by conductingpsychosocial assessments, counseling patients and families, coordinatingcommunity resources, and supporting emotional, financial, environmental, andend-of-life needs.

What You'll Do

Key Responsibilities

• Conductpsychosocial assessments to identify patients' and families' emotional,social, financial, environmental, and resource needs
• Providecounseling and emotional support to patients and families navigatinggrief, loss, and end-of-life care
• Educateand facilitate advance care planning discussions with patients andfamilies
• Coordinatecommunity resources, referrals and support services to meet patient andfamily needs
• Developand support individualized care plans
• Collaboratewith physicians, nurses, chaplains, and the interdisciplinary team
• Maintainaccurate and timely clinical documentation
• Communicateeffectively with patients, families, and healthcare providers
• Participatein interdisciplinary team meetings and collaborate on patient careplanning
