Job summary

Concho Hearts Hospice in Conroe, TX is seeking a motivated Hospice Marketer/Community Liaison to expand our presence in Conroe and surrounding communities. This full-time role focuses on building referral relationships with physicians, hospitals, and care facilities to grow patient admissions and awareness of hospice services.

You'll travel locally, educate healthcare professionals, and collaborate with clinical teams to ensure compassionate, seamless referrals and patient experiences.

Qualifications

• Minimum hospice or healthcare marketing experience required.
• Experience building referral relationships with physicians, hospitals, and facilities.
• Excellent communication, networking, and relationship-building skills.
• Self-motivated, organized, and able to work independently.
• Confident in presenting information to healthcare professionals.
• Ability to identify growth opportunities and execute marketing plans.

What You'll Do:

• Build and maintain strong relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners.
• Identify new referral opportunities and develop strategies to grow hospice awareness and admissions.
• Represent Concho Hearts Hospice throughout Conroe and surrounding communities.
• Educate healthcare professionals and community partners about hospice services and eligibility.
• Conduct regular visits and follow-ups with referral sources.
• Develop and maintain a consistent marketing schedule and territory plan.
• Collaborate with clinical and leadership teams to ensure a positive referral and admission experience.
• Participate in community events, networking opportunities, and educational presentations.
• Track referral activity, maintain accurate records, and communicate marketing progress.
• Meet established marketing and growth goals while maintaining professional and ethical referral practices.

What We're Looking For:

• Previous hospice or healthcare marketing experience required.
• Experience developing referral relationships with physicians, hospitals, and healthcare facilities.
• Strong communication, networking, and relationship‑building skills.
• Self‑motivated, organized, and comfortable working independently.
• Confident presenting information and educating healthcare professionals.
• Ability to identify growth opportunities and follow through on marketing goals.
• Positive attitude, professional appearance, and a genuine passion for helping others.
• Familiarity with Conroe and surrounding healthcare communities is a plus!
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout the assigned marketing territory.
