Job summary

Concho Hearts Hospice is seeking a Hospice Marketer / Community Liaison to build relationships within Fort Worth and surrounding areas. This full-time role focuses on educating partners, expanding referrals, and coordinating with clinical teams to ensure smooth admissions.

You will visit hospitals, skilled nursing facilities, and home health agencies, delivering outreach and education while growing hospice census.

Responsibilities

• Build and maintain relationships with hospitals, physicians, SNFs, ALFs, home health agencies, and other healthcare partners.
• Develop new referral sources and strengthen existing relationships.
• Make regular visits to referral partners in your territory.
• Educate healthcare professionals about hospice services and eligibility.
• Identify opportunities to increase referrals and grow census.
• Conduct community outreach, networking, and educational presentations.
• Follow up with referral sources and maintain strong relationships.
• Collaborate with clinical and leadership teams to ensure smooth referrals and admissions.
• Represent Concho Hearts Hospice professionally in the community.
• Track marketing activity, referrals, and growth opportunities.

What You'll Do:

• Build and maintain relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, home health agencies, and other healthcare partners.

• Hospice or healthcare marketing experience preferred.
• Proven ability to develop relationships and generate new business.
• Existing healthcare and community relationships are a plus.

• Reliable transportation, valid driver's license, and current auto insurance.
