Job summary

Halcyon Home LLC in Texas is seeking a dedicated Hospice LVN On-Call to provide triage and clinical support to hospice patients and families. This role follows a 7 days on, 7 days off schedule, with competitive pay and strong team support.

The LVN must hold an active LVN license, have at least 2 years in hospice, palliative, or acute care, and be adept at phone-based care, EMR usage, and coordinating care with on-call teams. HomeCare HomeBase experience is a plus.

Qualifications

• Active, state license as a licensed vocational/practical nurse (LVN)
• Minimum of 2 years of hospice, palliative care, or acute care experience; triage experience preferred
• HomeCare HomeBase experience is a plus

Responsibilities

• Conduct thorough triage assessments of patient symptoms and care needs via telephone
• Provide symptom management guidance and education to patients, families, and caregivers
• Coordinate with on-call hospice team members to facilitate timely care
• Ensure documentation and follow-up in the patient care system
• Offer emotional support and resources to patients and families
• Evaluate urgency of care needs and elevate cases when appropriate
• Communicate with interdisciplinary teams to update care plans
