Job summary

Halcyon Home is seeking a Hospice RN On-Call in Austin, TX to support our geriatric hospice population with 7 on & 7 off shifts. You will assess patients, coordinate care, and provide clinical guidance by phone while supporting Home Health and Home Care services.

Expect to work with a compassionate team, leveraging hospice principles, EMR proficiency, and strong communication in high-stress situations to deliver quality end-of-life care.

Qualifications

• Current RN license in good standing with state board.
• Minimum 2 years hospice/palliative or acute care experience.
• HomeCare HomeBase experience is a plus.
• Strong communication and critical thinking skills.
• Proficiency with electronic medical records (EMR).

Responsibilities

• Conduct thorough triage assessments of patient symptoms and care needs.
• Provide symptom management guidance and education to patients and families.
• Coordinate with on-call hospice team to ensure timely care.
• Document follow-up care and maintain accurate records in the EMR.
• Offer emotional support to patients and families through end-of-life care.
• Evaluate urgency and escalate cases when needed.
• Communicate with interdisciplinary teams to update care plans.

This role is essential in delivering compassionate end-of-life care, coordinating care plans, and assisting in urgent situations. The Hospice RN will assess patients’ needs, prioritize care, and offer clinical guidance over the phone. This position also supports Home Health and Home Care, and requires an RN with excellent communication skills, critical thinking abilities, and a deep understanding of hospice principles.

Responsibilities

• Conduct thorough triage assessments of patient symptoms, needs, and care requirements.
• Provide symptom management guidance, support, and education to patients, families, and caregivers.
• Coordinate with on-call hospice team members, including nurses, physicians, and other healthcare professionals, to facilitate timely care.
• Ensure appropriate follow-up care and documentation, keeping detailed and accurate records in the patient care system.
• Offer emotional support and resources to patients and families, guiding them through the end-of-life care process.
• Evaluate the urgency of care needs and elevate cases when appropriate.
• Communicate effectively with interdisciplinary teams to address patient needs and update care plans.

Requirements

• Current RN license and good standing with the state nursing board.
• Minimum of 2 years of hospice, palliative care, or acute care experience, with prior triage experience preferred.
• HomeCare HomeBase experience is a plus!
• Strong communication, problem-solving, and critical thinking skills.
• Proficiency in electronic medical record (EMR) systems.
• Ability to work independently, with empathy and professionalism, in high-stress and emotionally sensitive situations.
• Must have a reliable transportation
