The LPN/LVN - Hospice is responsible for providing care,comfort and support to terminally ill patients. The LPN/LVN - Hospice assumesthe responsibility and accountability for the delivery of hospice patient carein various settings. The LPN/LVNdemonstrates the ability to make clinical judgments and critical thinking in aneffective and efficient manner with supervision to perform patient care.

Minimum Requirements:

• Current Licensed Practical Nurse/Licensed Vocational Nursefor the state in which the nurse practices

• Complies with all relevant professional standards ofpractice

• Current CPR if applicable

• TB questionnaire, PPD or chest x-ray if applicable

• Current Health certificate (per contract or stateregulation)

• Must meet all federal, state and local requirements

• Must be at least 18 years of age
