Overview

Responsibilities

1. Report directly to the Administrator or Executive Director.
2. Provide direct patient care to hospice patients and support to their families, managing a reasonable caseload.
3. Evaluate patient and family needs and assist in developing and implementing the care plan.
4. Report changes in patient condition to the RN, interdisciplinary team, and physician, and document all reports.
5. Participate in staff meetings.

Qualifications

• Compassionate, kind, and empathetic nature.
• Comfortable providing care in patients' homes.
• Ability to remain flexible and composed during crises.
• Licensed Practical Nurse (LPN) license in the state of employment.
• At least 1 year of patient care experience in hospice, home health, ICU, geriatric, or oncology settings preferred.
• Additional experience in hospital or emergency settings is a plus.
• Valid driver’s license and auto insurance.
• Current CPR certification.
