Responsibilities

• Communicate health and nursing needs to RN supervisor
• Coordinate admissions, discharges, and follow-ups
• Assist with medical/health appointments
• Provide nursing care under RN supervision and physician orders
• Record vital signs, weights, wound measurements, etc.
• Identify trends requiring attention and communicate with RN/physician

Qualifications

• Licensed Practical/Vocational Nurse (LPN or LVN) in good standing
• CPR certification
• Minimum six months of direct nursing experience
