Capital City Hospice is seeking a dedicated LPN to join our Ohio team. You will deliver compassionate, hands-on hospice care, coordinate with the interdisciplinary team, and support patients and families in their homes.

Our approach centers on patient wishes, comfort, and quality of life. Expect a schedule of three 12-hour shifts, including every other weekend, with a patient-centric culture, good benefits, and opportunities for growth.

Qualifications

• Current CPR certification is required.
• Strong communication and organizational skills.
• Graduate of an accredited practical or vocational nursing program.

Responsabilités

• Provide compassionate, hands-on care to hospice patients.
• Coordinate services with the interdisciplinary team.
• Educate patients and families and manage symptoms.
• Support patients at home wherever they call home.

What You’ll Do:

As an LPN, you’ll provide compassionate, hands‑on care to hospice patients while supporting their families and caregivers. You’ll work closely with our interdisciplinary team to coordinate services, provide education, manage symptoms and help patients remain wherever they call home throughout their journey.

Qualifications Include

• Current LPN/LVN license
• Graduate of an accredited practical or vocational nursing program
• Three years of nursing experience preferred
• Hospice, community health, or medical/surgical experience preferred
• Current CPR certification
• Valid driver’s license and insured vehicle
• Strong communication and organizational skills
