Job summary

Good Samaritan is seeking a compassionate Licensed Practical Nurse (LPN) to join our mission-driven hospice team in the Roanoke area. You will provide direct patient care in homes and facilities, follow a patient-specific plan of care, and collaborate with RNs and other professionals to ensure comfort and quality of life.

You will document care in the EMR, educate patients and family caregivers, and participate in ongoing education and quality improvement initiatives.

Qualifications

Graduate of an accredited Practical Nursing program.
Current Virginia LPN license.
Minimum of three (3) years nursing experience; hospice or home health preferred.
Experience with electronic medical records (EMR).
Valid driver license and reliable transportation.
Current CPR certification (or ability to obtain).

Responsibilities

Provide compassionate, direct patient care under RN supervision per plan of care.
Deliver treatments, monitor conditions, and report changes to the RN Care Manager.
Assist with pain and symptom management to promote comfort and quality of life.
Educate patients and families about diagnoses, medications, and plan of care.
Document all care, assessments, and interventions in the EMR.
Collaborate with physicians and interdisciplinary team to coordinate hospice care.
Prepare and use medical equipment following infection control standards.
Participate in team meetings, continuing education, and quality initiatives.
Support mission with compassionate, patient-centered care in every interaction.
Participate in nursing holiday rotation.
