Job summary

Capital City Hospice in Columbus, OH, is seeking a compassionate Licensed Practical Nurse to join a dedicated hospice team. Provide direct patient care under the State Nurse Practice Act, complete assessments, document services, and educate families about care plans.

You bring an LPN license, CPR certification, at least 1 year of medical/surgical experience (home health or hospice preferred), and a reliable vehicle.

Qualifications

  • Graduate of an accredited practical or vocational nursing program.
  • Current LPN license in Ohio.
  • At least 1 year of medical/surgical experience (home health or hospice preferred).
  • Current CPR certification.

Responsibilities

  • Provide direct patient care in accordance with the State Nurse Practice Act.
  • Complete comprehensive assessments and follow the individualized plan of care.
  • Document patient services accurately and in a timely manner.
  • Educate patients and families/caregivers about diagnoses and care plans.
  • Collaborate with the interdisciplinary team (aides, OT, PT, MSW, Dietitian, Clinical Supervisor).
  • Manage pain and symptoms effectively and evaluate patient response.
  • Support patients and families with compassion, professionalism, and respect.

Schedule
  • Monday–Friday
  • 8 a.m.–5 p.m.
  • Only 2 holiday on-call shifts per year

What You’ll Do
