Ennoble Care is seeking a full-time Hospice Licensed Practical Nurse to serve patients in the North Philadelphia, PA area, delivering compassionate end-of-life care at home. You will work with the RN to implement the plan of care, administer medications, and provide emotional support to patients and families.

The ideal candidate is a graduate of an accredited Practical Nursing program, licensed in VA, with hospice experience preferred, strong communication skills, a valid driver's license, and a

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Licensed in VA as an LPN (Virginia State Board of Nursing).
• One+ years nursing experience; hospice experience preferred.
• Must have a valid driver's license with insured vehicle.
• Excellent verbal and written communication.
• Proven hospice LPN experience; safety and ethics focused.
• Compassionate with emotional stability; bilingual a plus.

Responsibilities

• Assist the Registered Nurse in carrying out the plan of care.
• Assist the Registered Nurse in performing specialized procedures as directed by the RN under physician's orders.
• Assist with the preparation, implementation, and continuing evaluation of the patient plan of care.
• Actively participate with the IDG to ensure the plan of care is current and issues are addressed.
• Help reinforce teaching to patient/family regarding end of life care.
• Foster cooperative effort and participate in team and staff conferences.
