Job summary

Transitions Hospice in Pekin, IL is seeking an experienced ECT Nurse to join our extended care team. This PRN role supports patients and families by providing flexible, skilled nursing coverage across multiple locations within our coverage territory.

As an ECT Nurse, you will assess, treat, and communicate patient needs with speed and clinical precision, administer medications, and collaborate with the interdisciplinary team to ensure timely, compassionate care.

Qualifications

• Active LPN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical hospice or home health experience preferred.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Assess patients and families and contribute to individualized care plans.
• Complete all required visit types to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses and respond with appropriate actions.
• Document timely clinical notes including pain and symptom management.
• Communicate changes in patient condition to physicians and obtain orders.
• Coordinate with community health partners to support care transitions.
• Ensure equipment and services are arranged and available.
• Maintain and grow your clinical skills through continuing education and in-service participation.
