Job summary

Transitions Hospice is seeking an experienced RN for Extended Care Team (ECT) nursing in Champaign and surrounding areas. You will join an interdisciplinary care team, providing flexible, hands-on nursing support across cases to assess, treat, and communicate patient needs with speed and clinical precision.

Expect varied assignments, a shift schedule, and a commitment to care that goes above and beyond. A valid RN license and hospice/home health experience are preferred, with local travel

Qualifications

• Must have active RN license in the employing state.
• Graduate of an accredited School of Nursing is required.
• Clinical experience in hospice/home health preferred.

Responsibilities

• Deliver professional nursing care across the full nursing process.
• Assess patients and families; contribute to care plans.
• Complete all visit types as needed to meet patient/family needs.
• Administer medications and treatments per orders.
• Document accurately; monitor pain and symptoms.
• Communicate changes in condition to physicians and obtain orders.
• Coordinate with community partners for seamless transitions.
• Ensure equipment and services are arranged for patients.
• Maintain and grow clinical skills through ongoing education.
