Job summary

PASCO/SW Home Health & Hospice is hiring a Licensed Practical Nurse (LPN) in Colorado to provide hands-on patient care in home settings under the supervision of an RN. You will implement care plans, document progress, and collaborate with a care team to ensure comfort and safety for patients living at home.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in the State of Colorado.
• 3 years of nursing experience preferred (Home Health or Med-Surg a plus).
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Deliver direct patient care in accordance with state regulations and care plans.
• Document progress and communicate observations to RN and physician.
• Collaborate with the care team to ensure high-quality service.
• Participate in professional development and team meetings.
