Job summary

EveryStep in Des Moines is seeking a Licensed Practical Nurse (LPN) to provide hospice nursing care in homes or facilities under the supervision of an RN. The LPN administers care per physician orders and supports the interdisciplinary care plan within hospice policies.

Responsibilities include direct patient care, medication administration, documentation, and family education. Travel between locations and on-call duties may be required.

Qualifications

• LPN licensed and in good standing.
• Hospice or home health experience preferred.
• Current BLS certification.

Responsibilities
