Job summary

Principle Choice Home Healthcare & Hospice is seeking an LPN to provide day-to-day nursing care for hospice clients in Oklahoma City. You will administer medications, monitor vital signs, and support patients and families through end-of-life care under RN supervision.

The role requires Oklahoma LPN licensure, BCLS, and a compassionate approach. On-site work with a collaborative interdisciplinary team, charting in the EMR, and adherence to HIPAA are essential.

Qualifications

• High School Diploma or equivalent.
• Currently licensed as an LPN in Oklahoma.
• BCLS current; ACLS preferred.
• Hospice LPN experience 2–3 years preferred.
• Driver's license and reliable transportation.
• Commitment to patient confidentiality under HIPAA.

Responsibilities

• Provide skilled nursing care to hospice patients under RN supervision.
• Implement care plan established by RN Case Manager and interdisciplinary team.
• Record vital signs and patient data.
• Observe, report, and document changes in patient condition.
• Administer medications and treatments per physician orders and policy.
• Educate patients and caregivers on medications, safety, and comfort measures.
• Assist with pain and symptom management.
• Provide catheter, wound, and ostomy care within scope of practice.
• Maintain timely EMR documentation and participate in care coordination.
• Support patients and families emotionally during end-of-life care.
