Hospice of the Piedmont is hiring a Licensed Practical Nurse to provide compassionate hospice care in senior living communities in or around Albemarle County, Virginia. You will collaborate with an RN, build relationships with patients and families, and ensure comfort, dignity, and quality end‑of‑life care.

You will document care in the EMR, support physicians and RNs, and participate in interdisciplinary teamwork. Frequent travel may be required as part of rounds and community visits.

Competenze

• Licensed LPN in the Commonwealth of Virginia.
• Two years’ nursing experience preferred.
• Ability to manage terminal care and coordinate with the health team.
• Basic to intermediate word processing skills.

Mansioni

• Deliver nursing services per Hospice policies.
• Document care using EMR.
• Assist physicians/RNs with procedures.
• Administer medications and treatments as prescribed.
• Observe, record, and report patient conditions to RN.
• Follow Hospice Plan of Care for patient/family services.
• Explain Hospice services to patients and families.
• Communicate with Interdisciplinary Team for coordinated care.
• Participate in QA programs and required meetings.
