Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Hospice Licensed Practical Nurse (LPN) to deliver patient-centered, high-quality care in Central Palm Beach County. You will work 8:30am - 5pm with weekend availability, under RN and physician supervision, and contribute to Full Life Care for all.

Florida LPN licensure, BLS, and a passion for hospice are required; 30-hour IV course is a plus. Join a mission-driven team dedicated to dignity and comfort at every life stage.

Qualifications

• Current license to practice as an LPN in Florida.
• Current Basic Life Support certification (BLS).
• 30 hour IV course a plus; hospice care interest considered.

Responsibilities

• Assess patient and family needs under RN and physician supervision.
• Coordinate and provide individualized care plans prioritizing comfort and dignity.
• Use monitoring, intervention, and teaching to improve quality of life.
• Provide emotional support to patients and families during overnight care.
• Deliver exceptional, personalized service for every patient and family.
