Job summary

Miracle City Hospice is hiring an LPN Continuous Care Nurse to support Broadview Heights and surrounding counties. You will provide direct patient care, follow the plan of care, and document services accurately while helping patients and families cope with a terminal illness.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience; hospice or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the practicing state.
• Must possess current CPR Certification.
• Demonstrates good verbal and written communication, and organization skills.
• Valid driver’s license with an automobile that is insured in accordance with state requirements.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Implement current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications.

What You’ll Do
