Job summary

Enhabit Home Health & Hospice in Georgia seeks a licensed Master of Social Work professional to administer social services and educate patients and families within the care team. You will work under physician orders and the approved plan of care in a home health setting.

Qualified candidates have an MSW from a CSWE-accredited program, licensure in their state (LMSW/LCSW), and at least one year of health care social work experience, plus basic technology skills.

Qualifications

• Master's degree from a school of social work accredited by the Council on Social Work Education is required.
• Must be certified or licensed in the state where they practice.
• Minimum of one year of social work experience in a health care setting.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

Education

Master of Social Work degree (CSWE-accredited)

State licensure as LMSW/LCSW or equivalent
