Job summary

Enhabit Home Health & Hospice seeks a licensed Master of Social Work to join the field-based care team in Texas. You will administer social services, educate patients and families, and collaborate with physicians to implement the plan of care.

The role requires a CSWE-accredited MSW, state licensure, at least one year of health-care social work experience, and CPR certification. A compassionate, patient-centered approach is essential in delivering high-quality home-based care.

Qualifications

• Must have a master's degree in social work from a CSWE-accredited program.
• Must be licensed in the state of practice (LSW/LMSW/LCSW).
• At least one year of social work experience in a health care setting.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services and educate patient, family and care team.
• Perform services in accordance with physician orders and the established plan of care (POC).
