Job summary

Lifecare-Home-Health-Family-1 is seeking a licensed Master of Social Work to supervise Medical Social Workers in a hospice setting. The MSW will support patient and family needs, participate in IDG/Team meetings, coordinate care, and ensure compliance with practice standards.

Travel to patient homes is required; on-call rotation, education, and documentation are part of the role. A MSW with Texas licensure and jurisprudence exam is preferred; experience in palliative care is valued.

Qualifications

• MSW licensure in Texas as LMSW.
• Master’s degree in social work.
• Compliance with Texas Behavioral Health standards.
• Pass Texas Jurisprudence Exam.
• Valid driver’s license.
• 1 year hospice/palliative care experience preferred.
• Strong knowledge of community resources.
• Good interpersonal and computer skills.

Responsibilities

• Supervise Medical Social Workers and coordinate with the IDG/T.
• Be readily available when social services are provided.
• Confer with MSW regarding patient care.
• Determine conference frequency per practice standards.
• Perform Comprehensive Assessments within 5 days of hospice election.
• Assist with advance directives, discharge planning, and family counseling.
• Maintain continuing education and QA participation.
• Provide end-of-life counseling and coordinate with IDG/Team meetings.
• Ensure patient privacy and proper documentation.
• Attend on-call rotations and meet productivity standards.
