Shift Information

This position is Full Time working Monday-Friday 8am-5pm.

Currently offering a $7,500 sign on bonus for full time LVNs.

What You Will Do

The Hospice LVN is a professional nurse who collaborates with the provider, IDG team and the supervising RN for hospice patients in their home environment (private residence, skilled nursing facility, assisted living, hospital, etc.) based on individual needs. The LVN provides patient care based on supervising RNs delegation, providers’ orders, departmental policies and procedures and regulatory guidelines and best practices. The LVN documents in the patient’s medical record in a timely manner according to agency policy. The LVN attends and actively participates in team meetings. The LVN performs on-call responsibilities and holiday staffing.

Required Education

Current Licensed Vocational Nursing Degree

Required Skills

Nursing skills as definted by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.) Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illness. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.

Preferred Skills

Experience with Home Care Home Base; Experience with smart phones and smart devices

Required Licensure/Registration/Certification

Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation.

Preferred Licensure/Registration/Certification

Wound Care certified; Advanced education or certification in area of specialty.

Required Experience

One year experience in a healthcare setting in respective discipline.

Preferred Experience

Medical-Surgical experience; Experience in Home Care or Hospice.
