Job summary

Care 360 Hospice is seeking a part-time Licensed Practical Nurse for after-hours coverage in Franklin County, Ohio. The role involves providing skilled nursing care to end-of-life patients in facilities or homes, with Saturday night shifts from 8 PM to 8 AM.

Ideal candidates have an LPN degree and at least 1 year of skilled care experience, with hospice/home health background preferred. Valid driver's license and reliable transportation are required, with willingness to travel within the service

Hospice Licensed Practical Nurse After-hours

Salary: $31.25-$34.13 BOE

Schedule: Part-time | Saturdays | 8 PM-8 AM

Location: Franklin County, Ohio, and surrounding area

Requirements of the Hospice LPN role

• Licensed Practical Nursing Degree
• A minimum of 1 year of skilled care experience
• Hospice Or Home Health Experience Preferred
• Valid driver's license and Auto insurance Coverage
• Reliable transportation: Willing to travel

About the Hospice LPN role

• Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
• Provide on-call services after hours, and complete skilled visits.
• Assist Registered nurses in carrying out the plan of care.
• Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders.
• Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
• Assist in the reinforcement of patient and family education regarding end-of-life care.
• Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
• Keep the RNCM informed of changes noted during the process.
• Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
