Job summary

Compassus Hospice is seeking a Licensed Practical Nurse / LPN to join our team in Savannah, GA. You will provide direct, skilled nursing under the RN Case Manager, delivering compassionate care to patients at home and supporting families in the hospice journey.

The role emphasizes building trusting relationships with patients, educating caregivers, and collaborating with the care team to ensure comfort and dignity at every step. LPN/LVN license is required to practice in Georgia.

Qualifications

• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license required.
• LPN/LVN must be active and in good standing.

Responsibilities

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy.
• Provide direct, skilled nursing services in accordance with the Plan of Care under the direction of the RN Case Manager.
• Build trusting compassionate relationships with your patients and their families.
