Part-Time LPN – Advantage Hospice

State College, PA | Flexible Hours | Compassionate Care

Are you a Licensed Practical Nurse seeking a meaningful role where your care truly matters? Advantage Hospice is currently hiring a Part-Time LPN to join our dedicated team in State College, PA. In this role, you’ll deliver hands-on nursing care and comfort to terminally ill patients under the supervision of a Registered Nurse, following a patient-centered plan of care developed by the interdisciplinary team.

Responsibilities

What You'll Do:

• Provide compassionate nursing care and symptom management to hospice patients
• Assist with treatments, medications, and preventive procedures as directed
• Monitor and report changes in patients’ physical or emotional condition
• Educate and support patients and their families on care and comfort measures
• Maintain accurate clinical documentation
• Participate in care planning, IDG meetings, and ongoing staff development
• Take on-call shifts including nights, weekends, and holidays as scheduled

Qualifications

• Graduate of a state-approved practical/vocational nursing program
• Active LPN license in Pennsylvania
• Minimum of 1 year of nursing experience preferred
• Hospice experience a plus, but training provided
• Commitment to compassionate end-of-life care and team collaboration
