Job summary

Liberty Hospice in North Carolina is seeking an experienced Hospice Licensed Practical Nurse (LPN) for a full-time position. You will provide direct patient care, document thoroughly, and follow the plan of care under the supervision of the Registered Nurse.

Primary duties include coordinating services with an interdisciplinary team, transcribing orders, and attending IDT meetings to ensure compassionate, state-of-the-art hospice care.

Qualifications

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state where the branch is located and one year of clinical nursing experience, preferably in an acute care setting.
• Ability to independently organize work and provide effective health care services to hospice patients.
• Excellent written and oral communication skills.
• CPR certification is required.

Responsibilities

• Provides direct patient care with a focus on comfort and symptom management.
• Completes documentation as required by policy and regulation.
• Follows the plan of care developed by the RN.
• Coordinates hospice services with the RN and other staff to optimize patient care.
• Accepts and transcribes physician orders as directed.
• Forwards clinical notes to the RN for coordination of care.
• Attends and participates in IDT meetings.

Job Description

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates Hospice services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Hospice Registered Nurse for coordination of care.
• Attends and participates in IDT meetings.

Job Requirements

• Ability to independently organize work and provide effective health care services to hospice patients.

• Effective communication skills (oral and written).
• CPR certification.
