Job summary

Liberty Hospice in North Carolina is seeking a dedicated Hospice Licensed Practical Nurse (LPN) to join our full-time team. You will provide direct patient care in a hospice setting, follow the plan of care, and coordinate with the RN and care colleagues to ensure comfort and quality at the end of life.

The ideal candidate has an approved nursing diploma, NC LPN licensure, CPR, and at least one year of clinical experience.

Qualifications

• Successful completion of nursing education from an approved school.
• Current LPN licensure in North Carolina.
• At least 1 year of clinical nursing experience, preferably in an acute care setting.
• Ability to independently organize work and provide effective health care services to hospice patients.
• Strong communication skills (oral and written).
• CPR certification.

Responsibilities

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates Hospice services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Hospice Registered Nurse for coordination of care.
• Attends and participates in IDT meetings.

• Ability to independently organize work and provide effective health care services to hospice patients.
• Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
