Job summary

Liberty Health in Blowing Rock, NC seeks an experienced Hospice Licensed Practical Nurse (LPN) for a part-time role. The LPN will provide direct patient care, document thoroughly, and work closely with the RN and care team to coordinate hospice services.

Strong communication and CPR certification are required. Candidates should have an active North Carolina LPN license and at least one year of clinical nursing experience; acute care or hospice experience is preferred.

Qualifications

• Current LPN licensure in North Carolina.
• At least one year of clinical nursing experience, preferably in hospice or acute care.
• Ability to organize work and document care thoroughly.
• Good oral and written communication skills.
• CPR certification is required.

Responsibilities

• Provide direct patient care and coordinate services.
• Complete documentation per policy and regulatory requirements.
• Follow the plan of care developed by the RN.
• Coordinate hospice services with the RN, care coordinator, and other staff.
• Accept and transcribe physician orders as directed.
• Forward notes to RN for coordination of care.
• Attend and participate in IDT meetings.

Job Description:

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient's diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates Hospice services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician's orders as directed.
• Forwards all clinical, progress, and summary notes to the Hospice Registered Nurse for coordination of care.
• Attends and participates in IDT meetings.

Job Requirements:

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to hospice patients.
• Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
