Job summary

AccentCare is seeking a Hospice RN at the Inpatient Center in Naperville, IL. This full-time night role runs 7pm–7am, three 12-hour shifts weekly with every-other-weekend rotation.

The RN will provide comprehensive hospice nursing care, coordinate with the care team, and ensure proper documentation while supporting patients and families. Qualified candidates are licensed RNs; hospice/home health or medical-surgical experience is preferred.

Qualifications

• Must be a licensed RN in the state of practice.
• Hospice, home health, medical-surgical, or population-specific care experience preferred.
• Willingness to serve on-call and uphold the organization’s values.

Responsibilities

• Provide comprehensive nursing care by assessing, planning, implementing, and evaluating hospice patient plans.
• Coordinate with physicians and the interdisciplinary team for seamless care.
• Maintain timely documentation and update patient records during shifts.
• Educate patients and caregivers on medications, mobility and end-of-life care.
• Provide emotional support and participate in 24/7 care coverage with the team.
• Attend meetings and assist with training and compliance with regulations.
