Job summary

JourneyCare Hospice in Naperville, IL is seeking an experienced Hospice RN to join our growing team. This full-time weekend shift offers 32 hours per week with competitive salaries and comprehensive benefits.

You will assess patients, coordinate with the interdisciplinary team, perform death pronouncement when needed, and maintain accurate care plans and notes. A state RN license and CPR certification are required, plus a valid driver’s license.

Qualifications

• Graduate from an accredited registered nursing program.
• Current license to practice as a Registered Nurse in the state.
• Minimum of two years of varied work experience as a professional nurse.
• CPR Certification.
• Valid driver’s license and proof of insurance.

Responsibilities

• Obtain data on patient and family needs and initiate interventions with the interdisciplinary team.
• Coordinate care by communicating with the attending physician and other staff.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate records with care plans and progress notes.
