Job summary

Circle of Life is seeking an Intake Nurse to coordinate admissions to Hospice services in a fast-paced setting. The role requires handling referrals, obtaining patient records, and maintaining compassionate communication with patients and families.

Eligible candidates hold an unrestricted RN or LPN license and have at least 2 years of nursing experience in home health or healthcare facilities. Flexibility for weekend/holiday rotations is essential.

Qualifications

• Unrestricted RN or LPN license required.

• Must be flexible to change schedule on short notice.
• Exemplary clinical skills and problem-solving ability.

Responsibilities

• Coordinate and execute admission process to Hospice services.
• Obtain referral orders and patient records.
• Provide phone support to patients, families, and referral sources.
• Document care navigation and palliative referrals in RM and WellSky.
• Schedule APRN or physician face-to-face appointments.
• Maintain timely communications with physicians, clinics, SNFs/ALFs.
