Job summary

Parker Jewish Institute's Community Health Services is seeking an experienced Intake Coordinator to oversee the intake function for hospice and home health programs. You will ensure patients are appropriate for services and evaluated timely by home care and hospice staff.

The role supports program growth, delivers quality patient care, and collaborates with an interdisciplinary team in a not-for-profit setting with competitive benefits.

Qualifications

• 1–2 years of prior experience in a care coordinator, intake, admissions or community health role preferred.
• Experience in Home Care, Hospice or Community Health environment preferred.
• Proficient in EMR software; Netsmart familiarity a plus.

Responsibilities

• Oversee the intake function for Parker's hospice and home health programs.
• Ensure patients accepted to the program are appropriate and evaluated timely by staff.
• Collaborate with interdisciplinary teams to support growth and high-quality patient care.
