Job summary

Saint Francis Hospice in Tulsa, OK is seeking a full-time Registered Nurse for home health visits Monday–Friday 8:00 AM–4:30 PM, with flexible start times and minimal on-call.

The RN assesses patient needs, plans and implements care, coordinates with physicians and caregivers, documents in EMR, and ensures safe discharge planning. Requires Oklahoma RN license and reliable transportation.

Qualifications

• Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing.
• Holds or is entitled to hold a diploma or degree in nursing.
• Valid multi-state or State of Oklahoma Registered Nurse License.
• A valid driver's license and a Motor Vehicle Report that meets SFHS guidelines.
• Minimum 1 year of related experience preferred.

Responsibilities

• Performs assessment to determine patient eligibility and needs for home health services.
• Develops and revises a patient-centered plan of care based on patient goals and preferences.
• Delivers nursing care with consideration for cultural, psychological, functional, cognitive and social determinants.
• Includes patient and family in the care planning process.
• Organizes case management responsibilities including complex tasks.
• Documents patient care in EMR and communicates medication issues with the health care team.
