Job summary

Saint Francis Health System is seeking a Registered Nurse for the Yale Campus - Warren Clinic Tower in Tulsa, OK. This full-time hospice nurse role operates on a Monday–Friday days schedule with 8:00 AM–4:30 PM (flexible) and minimal call.

You will assess patients, develop care plans, coordinate with physicians and caregivers, review medications, and document in the EMR. Valid RN license in Oklahoma and a valid driver's license are required; one year of related experience is preferred.

Qualifications

• Completed basic professional nursing education and licensed as RN in Oklahoma.
• Valid multi-state or Oklahoma RN license and driver's license.
• Minimum 1 year of related experience preferred.

Responsibilities

• Assess patients and develop a patient-centered home health plan.
• Deliver nursing care and coordinate with health team and caregivers.
• Review medications for adverse effects and document in EMR.
• Ensure timely discharge planning and progress toward goals.
