Community Hospice and Palliative Care seeks a Licensed Practical Nurse (LPN) in a float role to deliver compassionate, high‑quality care across inpatient centers. The role covers 36 hours per week, with a 7 am–7 pm shift, and requires active Florida LPN licensure with IV and CPR certifications.

The LPN will collaborate with RNs and interdisciplinary teams, participate in discharge planning, and be responsible for medication administration, IV therapy where permitted, and patient education.

Formación

• Minimum two (2) years of patient care experience
• Current Florida LPN license
• IV Certification (Florida preferred)
• Current CPR certification
• Graduate from an accredited Florida Board of Nursing program for Licensed Practical Nurses

Responsabilidades

• Assist in implementing and modifying patient care plans with RN and team.
• Deliver patient-centered care, including administering medications and performing procedures within the LPN scope.
• Monitor and report changes in patient condition; document accordingly.
• Educate patients and caregivers on self-care practices.
• Participate in discharge planning and team meetings.
• Maintain patient confidentiality and HIPAA compliance.
• Manage and order medical supplies as needed.
• Facilitate communication between care team and physicians.
• Support organizational initiatives such as disaster preparedness and performance improvement.
• Administer IV therapy as permitted by licensure and training.
• Follow safety and infection control procedures.
• Demonstrate professional relationships with internal and external customers and promote the organization’s image.
• Use chain of command for problem solving and maintain attendance per policy.
• Participate in QAPI activities and follow Clinical Care standards
