Hospice of the Chesapeake in Waldorf, MD is seeking a compassionate RN to join our Inpatient Care Center as a GIP specialist. You will deliver direct nursing care, care management, and collaborate with social workers, chaplains, CNAs, and physicians to support end-of-life care for patients and families.

The role requires an MD RN license (Maryland or reciprocal), 2+ years in med/surg/home care with hospice experience preferred, CHPN preferred, and a valid driver's license.

Qualifications

• Current Maryland RN license or accepted reciprocal out-of-state nursing licensure.
• 2+ years of nursing experience in med/surg or home care; hospice or end-of-life care preferred.
• CHPN certification preferred.
• Ability to document thoroughly in an Electronic Medical Record and meet physical demands.

Responsibilities

• Provide direct patient care to meet total nursing needs per hospice plan of care.
• Collaborate with interdisciplinary team and families to develop and adjust care plans.
• Document care, treatments, and education at point of care or in corridors.
• Assist field teams as needed and ensure safe, compassionate end-of-life care.

# Hospice Inpatient Center RN/GIPHot JobCharles County - Waldorf, MD 20603## OverviewSalary Range$41.00 - $50.00 HourlyPosition TypeFull Time## Description**Want to join our Team?** We are currently looking for compassionate and dedicated RNs to join our Inpatient Care Center (ICC) team. As an RN in a GIP-setting, you will take the lead in providing nursing care and case management for those with life-limiting illnesses, helping patients and their caregivers navigate end-of-life care with compassion. You will work with your team’s Social Workers, Chaplains, and CNAs, as well as our Hospice Physicians, to provide the physical, psychosocial, and spiritual aspects of care. **Office Location: Waldorf, Maryland****Coverage Area: Charles County Inpatient Care Center****Status: Full-Time****Schedule: 7am - 7:30pm****Salary: Competitive, commensurate with experience, education and credentialing** **Main Responsibilities:*** Provide direct patient care sufficient to meet the patient’s total nursing need in accordance with the patient’s hospice plan of care assuring each patient receives treatments, medications, and diet as tolerated.* Correctly identifies, communicates and collaborates on patient problems/priorities/issues based upon assessment data, establishes measurable patient goals and communicates initial/ongoing plan of care appropriately to interdisciplinary team. Recognizes changes in clinical status and reports them to the appropriate individual.* Collaborates with patient/family when developing plan of care, adjusting communication style as necessary. Establishes priority for patient care based on immediate patient condition and mutually set patient needs/expectations.* Ensures patient/caregiver teaching and patient/caregiver level of understanding and response to teaching are documented. Incorporates teaching into routine care, involving patient/caregiver.
