Heart to Heart Hospice is seeking a dedicated Hospice Aide/CNA to provide direct patient care under RN supervision in Sherman, Texas. The role involves assisting with transfers, documentation, and compassionate support for patients and families in a home-like setting.

Ideal candidates will hold a current CNA license, have at least one year of related experience, and demonstrate integrity and a heart for caring for ill and elderly.

• High school diploma or GED required.
• Current CNA license is required.
• At least one year of hospice or nursing assistant experience preferred.
• Demonstrates heart for compassionate care and integrity.
• EMR experience is a plus.

Responsabilidades

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
