Job summary

Mission Hospice & Home Care in Redwood City is seeking a part-time Licensed Vocational Nurse (LVN) to provide direct patient care at the Hospice House. The role supports end-of-life care and education for patients and families.

Schedule is Monday, Thursday, Saturday, Sunday 7:00am–3:30pm. Local nurses must have CA LVN license; IV therapy certification preferred; driving may be required.

Qualifications

• Completion of a Licensed Vocational Nursing Education Program from an accredited school.
• Current California LVN license required.
• Valid California driver's license and auto insurance if driving.

Responsibilities

• Provides direct patient care under current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient care services.
• Assess and provide patient and family education and information pertinent to plan of care.
• Participates in the coordination of hospice services and report identified needs to the Program Director and interdisciplinary team.

We are seeking an LVN who will be responsible for providing direct patient care to hospice patients at our Hospice House in Redwood City.

Essential Duties & Responsibilities include (but are not limited to):

• Provides direct patient care under current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient care services.
• Assess and provide patient and family/caregiver education and information pertinent to plan of care.
• Participates in the coordination of hospice services and report identified needs to the Program Director and interdisciplinary team.

Education/Experience

• Completion of a Licensed Vocational Nursing Education Program from an accredited school
• Experience in a home health/ hospice setting preferred but not required

Certificates, Licenses, Registrations

• Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners.
• Valid California certification in Intravenous Therapy & Blood Withdrawal preferred
• Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Pay Range

$38.65 - $48.40 hourly
