Job summary

McLeod Health in Cheraw is seeking a Registered Nurse for Hospice Homecare. You will deliver compassionate, patient- and family-centered care and work with an interdisciplinary team to uphold our Service Excellence Standards.

Responsibilities include assessing patients, developing and implementing care plans, coordinating services, and participating in on-call rotations. Active SC RN license and BLS are required; one year of nursing experience preferred.

Qualifications

• Must have active patient contact within the past year.
• Current South Carolina RN License.
• At least one year of nursing experience; nursing refresher course within past year if applicable.
• Current South Carolina Driver’s License and Automobile Insurance as required by law.
• Current BLS certification (American Heart Association).

Responsibilities

• Maintain a professional image and exhibit excellent customer relations with patients, visitors, physicians, and co-workers.
• Assess patient needs based on referral evaluation and nursing assessment.
• Develop and implement a plan of care reflecting the physician's medical plan.
• Serve as case manager coordinating the interdisciplinary team and attending team conferences.
• Participate in on-call rotation and complete documentation per policy.
