Job summary

Hospice of the Piedmont, Inc. is seeking a Homecare RN Case Manager in Asheboro, NC. This full-time, salaried, exempt role follows a Monday-Friday, 8am-5pm schedule, serving patients within a 30-mile radius of the Asheboro office.

You will create and implement care plans, document visits, and coordinate with the Nurse Practitioner and IDG. The position emphasizes autonomy, compassionate patient/family communication, and collaborative team care.

Qualifications

• RN license required and valid driver's license.
• Associates or better in Nursing (required).
• Bachelor's or better in Nursing (preferred).

Responsibilities

• Schedule patients and document visits (assessments, interventions, updates).
• Develop and coordinate plan of care.
• Communicate with Nurse Practitioner.
• Average of 4 patient visits per day.
• Supervise CNA services for assigned patients.
• Call family to explain patient changes.
• Attend IDG meetings as an active member.
