Job summary

The Pennant Group in Utah is seeking an experienced Registered Nurse to plan, organize, and direct hospice care in home settings. You will assess patients and families, develop care plans, administer medications, and collaborate with an interdisciplinary team to ensure compassionate, quality care.

Candidates should have an accredited nursing degree, current state licensure, CPR, and hospice experience preferred.

Qualifications

• Graduate of an accredited school of nursing.
• One to two years of recent acute care experience in an institutional setting and one to two years of recent home care experience.
• Current licensure in state and CPR certification.
• Bachelor's degree, with one year hospice care experience preferred.
• Management experience not required; may supervise hospice aides.
• Excellent observation, verbal and written communication skills, problem solving skills and basic math skills.

Responsibilities

• Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using nursing process.
• Develops and revises plan of care based on patient status and needs; documents progress.
• Initiates plan of care and revisions as patient status changes; coordinates with care team.
• Uses health data to determine nursing diagnoses and goals in collaboration with patient/family.
• Administers medications and treatments as prescribed by physician.
• Counsels patient and family in meeting nursing and related needs; provides care instructions.
• Assists patient with activities of daily living and promotes self-sufficiency and comfort care.
• Acts as Case Manager when assigned; coordinates patient care for caseload.
• Maintains accurate clinical notes and communicates with physician and care team.
• Attends interdisciplinary group meetings and participates in care coordination.
