Job summary

Sentara Health Plans is seeking a Direct Care Registered Nurse to provide compassionate nursing care in patients' homes or hospice settings during the Evening shift. This role focuses on end-of-life care, ensuring comfort and dignity for patients and support for families.

The RN will perform assessments, coordinate with the care team, and deliver high-quality care on a full-time basis in South Boston, VA, with potential travel within ~50 miles. VA RN license and BLS are required.

Qualifications

• Associate's Degree in Nursing is required.
• Valid RN License in VA is required.
• BLS certification is required within 90 days of hire.
• 1 year of nursing experience is required.
• Previous home care/hospice experience is preferred.

Responsibilities

• Provide high-quality nursing care to patients in their homes or hospice settings, ensuring comfort, dignity, and emotional support.
• Conduct assessments and develop care plans in collaboration with the care team.
• Coordinate with physicians and other professionals to ensure comprehensive care.
