Job summary

HRAnswerLink seeks an experienced Registered Nurse to provide hospice care, evaluate patients, and implement current clinical practices in a collaborative setting.

You will plan care with physicians, monitor outcomes, and educate Home Health Aides and LPNs while benefiting from excellent compensation and continuing education opportunities.

Qualifications

• Current and unencumbered RN license for the state of employment.
• CPR certification is required.
• NLC knowledge and multi-state licensure flexibility.
• Ability to assess patient status and plan care accordingly.
• Reliable transportation with a valid driver's license.

Responsibilities

• Evaluate and treat patients using current technology and practices.
• Communicate with physician to decide initial plan of care from assessment and orders.
• Participate in clinical outcomes monitoring and CQI initiatives.
• Spend time with patients and make a meaningful difference.
• Educate Home Health Aides and LPNs on nursing services implementation.
• Collaborate with a multidisciplinary team and maintain patient interactions.
• Provide timely after-hours/weekend/holiday responses as needed.
• Demonstrate understanding of CQI processes and participate in CQI.

Job description

• Evaluate and treat patients using the most current technology and practices
• Communicate with the physician to decide the initial plan of care based on initial patient assessment and physician’s orders
• Participate in clinical outcomes monitoring, follow up and agency performance improvement initiatives
• Spend more time working with patients and making a significant difference in their lives
• Manage and educate Home Health Aides and LPNs performance in implementing nursing services

Requirements

• Current and unencumbered license to practice as a Registered Nurse specific to that state the employee is assigned to work
• Minimum of two (2) year of hopice experience preferred
• Current CPR certification
• Nurse Licensure Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state’s practice law and regulation (nurses must legally reside in a NLC state to be eligible to have a multi-state license)
• Able to assess patient status and identify requirements relative to age specific needs
• Reliable transportation and valid driver’s license with no DUIs, revocations, or suspensions within the past 5 years
