Job summary

Yale New Haven Health in Connecticut is seeking a Home Health Aide to provide direct patient care in home settings under the supervision of the RN Case Manager or Therapist. This role emphasizes integrity, patient-centered care, and compassionate service.

The ideal candidate will have CT HHA certification or enrollment in required training, a valid CT driver's license, and the ability to work with complex patients in home environments.

Qualifications

  • Certified according to State of Connecticut Public Health Code regulations.
  • Completion of a CT-DOH approved home health aide training program or enrollment in 75 hours of nursing theory/skills for CT DPH.
  • Successful completion of a CT DPH competency evaluation; renewal if 24 months have passed without services.
  • Must have a valid CT driver's license and a clean driving record.

Responsibilities

  • Provide direct patient care (bathing, dressing, oral care, toileting) per plan of care.
  • Assist complex patients (bedbound, palliative, hospice) as directed.
  • Ensure safe ambulation, transfers, and use of assistive devices with proper body mechanics.
  • Assist with ROM exercises and nurse/therapist prescribed exercises.
  • Assist with meal preparation per prescribed diet and fluid restrictions.
  • Help maintain patient environment cleanliness.
  • Communicate any concerns to RN Case Manager or therapist.
  • Follow universal precautions and PPE.
  • Document care and observations as instructed.
  • Ensure all patient documents are signed, dated, and submitted timely.
  • Conform to payroll standards and HIPAA.
  • Comply with weekday/weekend commitments.
  • Participate in staff meetings and ongoing trainings.
