Job summary

Yale New Haven Health seeks a Home Health Aide to deliver direct patient care in home settings under supervision of a nurse or therapist.

Responsibilities include assistance with bathing, grooming, mobility, meals, and documentation of care provided in coordination with the RN Case Manager. A CT driver’s license and vehicle are required, with lifting ability up to 50 pounds and weekend availability may be necessary.

Qualifications

• High School diploma or GED preferred.
• CT Home Health Aide certification or eligible.
• Valid CT driver’s license with clean record and own vehicle.
• Ability to lift up to 50 lbs and perform home care tasks.

Responsibilities

• Provides direct patient care (bathing, dressing, oral care, toileting) per plan of care.
• Assists with ambulation, transfers, and use of assistive devices.
• Prepares meals per prescribed diets and documents intake.
• Documents care and notifies RN Case Manager of changes.
• Maintains a clean patient environment and follows HIPAA.
